Flu Vaccination Clinic

Nursing Staff from AMITA Health will be administering Flu Vaccines at the Chamber Office from 10:00 a.m. to 1:00 p.m.

The vaccine will be provided free of cost to all patients other than Medicare Patients - No insurance card necessary. 
Insurance card required only for Medicare B patients (seniors).  AMITA will bill Medicare directly for services.  

Patients will sign a general consent form and “fee waiver”.
